California Medical License Application Services

A California medical license is issued by the Medical Board of California, the largest medical board in the country by licensee count, with correspondingly long queues. Applications are filed through the BreEZe online licensing system, with documents submitted electronically through DOCS and require 36 months of board-approved postgraduate training, including 24 consecutive months in the same program, for all applicants regardless of where they attended medical school; residents hold a Postgraduate Training License during training, and physicians licensed after 24 months must verify the full 36 months by their first renewal. The board also requires Live Scan fingerprinting, verification of education and every year of training sent directly from the institutions, and disclosure of any prior discipline or malpractice history. Every element is verified at the primary source before the file is considered complete.

On controlled substances, California does not issue a separate controlled substance registration; prescribers need a DEA registration and mandatory enrollment in the CURES prescription drug monitoring program. Renewal is biennial, expiring on the last day of the birth month, with 50 hours of approved CME per cycle and a one-time 12-hour course on pain management and the care of the terminally ill within four years of licensure; the active renewal fee is $1,206. California is not a member of the Interstate Medical Licensure Compact and offers no reciprocity, so every physician completes the full application, and there is no out-of-state telehealth registration, so a physician treating California patients by telehealth must hold a full California license. Institution-by-institution verification

California requires direct verification from every school and training program rather than relying on FCVS alone. Each institution has to be chased individually, and mailed documents are no longer accepted.

Training months counted wrong

The 36-month requirement, with 24 consecutive months in one program, is documented month by month. Leaves of absence and non-accredited rotations are excluded and can leave an applicant short at first renewal.
